What can a scar cream do?
A scar is the consequence of our own body healing a wound or injury inflicted to our skin or other tissues. Damage to the outside layer of the skin is healed by rebuilding the tissue, and in these instances, scarring is slight. When damage affects the thick layer of tissue beneath the skin, rebuilding gets more complicated. Our bodies lay down collagen fibres (a protein which is naturally produced by the body) to reconnect the skin tissues and this usually results in a noticeable scar.

After the wound has healed, the scar continues to change as new collagen is formed and new blood vessels are created. This is the reason why most scars will fade and improve in appearance over a period of two years following an injury. However, some visible evidence of an injury will remain as hair follicles and sweat glands do not grow back.

How Can Scar Creams Benefit The Healing of My Injuries?

There are many scar healing creams, ointments, patches of silicone and products made made with vitamins, botanical extracts, and natural ingredients that are designed to improve the healing of scars,  help in scar reduction and make scars less noticeable.

But not all those creams or products for scars are alike.

Healing of wounds or injuries is a process that entails coordinated orchestration between the many cells, organs and products of the immune system that do the masterful job of maintaining our biological integrity.

The Inflammatory Phase

Platelets and inflammatory cells are the first to arrive at a site injured and provide key functions and “signals” necessary for the influx of connective tissue cells and a new blood supply. This phase is named inflammatory phase and is characterized by redness, heat, swelling and pain.

The presence of wound macrophages is a marker that the inflammatory phase is nearing the end and a proliferative phase is beginning. Lymphocytes (white blood cell that helps the body fight infection) come into the wound area at a later stage.

This substance is a mucin secreted by land snails. A complex compound of molecules that comprise glycoprotein enzymes, glycosaminoglycans, proteoglycans, copper-haemocyanin and oligoelements that contribute to repair skin and affect every phase of the process that repairs skin damage and leads to minimum scarring or scarless healing.

Proliferative Phase

As the Proliferative phase progresses the predominant cells in the wound site are the fibroblasts. These cells originate in the mesoderm that gives rise to the connective tissue of the body, blood vessels, and lymph vessels and is responsible for producing the new matrix needed to restore structure and function to the injured tissue. Fibroblasts attach to the cables of the provisional fibrin matrix and begin to produce collagen.

At this stage BIOSKINCARE presents fibroblast growth factor-like activity with the following effects:

Facilitates the migration of keratinocytes
Enhances fibroblast functional capability, thus improving fibroplasia
Induces synthesis of new collagen
Provides hyaluronic acid (essential in all repair process)

The rapid production of hyaluronic acid by fibroblasts in the early stages of wound healing are of crucial importance as hyaluronic acid stimulates the migration and mitosis of mesenchymal and epithelial cells. Increased levels of hyaluronic acid, as observed during fetal wound healing or as achieved by the topical application of hyaluronic acid during wound dressing, are associated with brisker healing and reduced scarring.

Collagen is an abundant protein in the animal kingdom, accounting for 30% of the total protein in the human body. In normal tissues collagen provides strength, stability and structure. Collagen is needed to repair defects and restore anatomic structure and function. If too much collagen is deposited in the wound site, normal anatomical structure is lost, function is compromised and fibrosis occurs.

Remodelling Phase

BIOSKINCARE helps achieve the correct balance between the synthesis and degradation of collagen and other important structural elements.

The collagen released into the extracellular space undergoes further processing by opening of the procollagen N and C-terminal peptides. In the extra-cellular spaces an important enzyme, lysyl oxidase, acts on the collagen to form stable cross-links. As the collagen matures and becomes older, more and more of these intramolecular and intermolecular cross-links are placed in the molecules. This important cross-linking step gives collagen its strength and stability over time.

In the remodelling phase BIOSKINCARE induces the assembly of the fibronectin extracellular matrix and provides the necessary structural support for new collagen and elastic fibres. It also enhances cell communication and cell-matrix interactions, allowing proper matrix assembly during the repair process.

In summary, the normal healing cascade begins with an orderly process of hemostasis and fibrin deposition, which leads to an inflammatory cell cascade, characterized by neutrophils, macrophages and lymphocytes within the tissue. This is followed by attraction and proliferation of fibroblasts and collagen deposition, and finally remodeling by collagen cross-linking and scar maturation.

Despite this orderly sequence of events responsible for normal wound healing, pathologic responses leading to fibrosis or chronic ulcers may occur if any part of the healing sequence is altered. Those can be avoided with the application of BIOSKINCARE twice a day every day during the period of time the scar healing process is still at work.

A scar, or cicatrix in medical terminology, is a mark left in the skin by the healing of a wound, burnt or surgical incision in which the normal functional tissue (skin) is replaced by connective tissue (scar).

There are lots of different causes of scarring - accidents, surgery, skin disease, burns, acne, infection and crime - but not all scars are the same. Below are some of the different types of scars that occur.

Flat, Pale Scars
These are the most common type of scar and are a result of the body's natural healing process. Initially, they may be red or dark and raised after the wound has healed but will become paler and flatter naturally over time, resulting in a flat, pale scar. This process can take up to two years and there will always be some visible evidence of the original wound.

Hypertrophic Scars
(Red or Dark and Raised)
When a normal wound heals the body produces new collagen fibres at a rate which balances the breakdown of old collagen. Hypertrophic scars are red and thick and may be itchy or painful. They do not extend beyond the boundary of the original wound but may continue to thicken for up to 6 months. They usually improve over the next one to two years but may cause distress due to their appearance or the intensity of the itching, also restricting movement if they are located close to a joint.

Hypertrophic scars are more common in the young and people with darker skin. Some people have an inherited tendency to this type of scarring. It is not possible to completely prevent hypertrophic scars, so anyone who has suffered one should inform their doctor or surgeon if they need to have surgery. Scar Therapies are available which may speed up the process of change from a hypertrophic scar to a flatter, paler one.

Keloid Scars
Red or Dark and Raised)

Like hypertrophic scars, keloids are the result of an imbalance in the production of collagen in a healing wound. Unlike hypertrophic scars, keloids grow beyond the boundary of the original wound. They can continue to grow indefinitely. They may be itchy or painful and may not improve in appearance over time.

Keloid scars can result from any type of injury to the skin, including scratches, injections, insect bites and tattoos. Anybody can get a keloid scar and they can occur anywhere on the body. However, the young and people with darker skin are more prone to this type of scarring and they are more common on certain parts of the body e.g. ears, chest, shoulders and back.

As with hypertrophic scarring, people who have developed one keloid scar are likely to be prone to this condition in the future and should alert their doctor or surgeon if they are likely to need injections or to have any form of surgery.

Sunken Scars
Sunken scars are recessed into the skin. They may be due to the skin being attached to deeper structures (such as muscles) or to loss of underlying fat. They are usually the result of an injury.

Acne & Chicken Pox Scars
A common cause of sunken scarring is acne or chicken pox which can result in a pitted appearance. However, it is important to note that acne scarring is not always sunken in appearance and can even become keloid.

Stretched Scars
Stretched scars occur when the skin around a healing wound is put under tension during the healing process. This type of scarring may follow injury or surgery. Initially, the scar may appear normal but can widen and thin over a period of weeks or months. This can occur where the skin is close to a joint and is stretched during movement or may be due to poor healing due to general ill health or malnutrition.

Stretch Marks
Stretch marks develop when the skin is stretched rapidly, for example during pregnancy or the adolescent growth spurt. Initially, stretch marks appear red but become paler over a number of years.

Scar Psychology

Most people have at least one scar on their body and luckily, for the majority of us, it does not cause any real upset. However, through my work at the Wound Healing Research Unit, I am in contact with people who find that living with a scar is more of a problem to them. It's not just the person who has the scar(s) but also the family and friends who are affected.

One thing that has become very clear to me is that there are a wide range of things which influence how we feel about scars, including the location of the scar, our age and sex and how the scar occurred. Facial scars can be very distressing but other less visible scars can also be very upsetting.

Society places a great deal of importance on how we look. While scars are considered desirable in some societies and may even be inflicted deliberately, in the majority of cultures this is not the case. Often the "bad" character in a film is portrayed as having facial scarring which highlights the fact that our society not only views scars as undesirable but also perpetuates the myth that those with scars are likely to be less than pleasant people.

If we feel that people are reacting differently to us because we have a scar, it can cause embarrassment and a lack of self-confidence; some people eventually feel angry and isolated as a result of such experiences.

If you or someone you know is having problems coming to terms with a scar then do not despair. There are people who can help and ways you can help yourself.

Look into the scar therapies which are available but don't raise your expectations too high - no therapy will erase a scar completely.

It may take years to come to terms with a scar but it will help you accept your situation if you try to think positively - after all nobody is perfect and your scar is part of who you are.

Talk to other people who have a scar, your doctor or arrange some counseling sessions. This may help you to come to terms with your scarring and does not mean that you will be referred to a psychiatrist. Remember that it can be quite normal to feel distressed about a scar and there are a number of organizations which may be able to help (see useful addresses)

What should you do if you have a problem scar?

Scars usually take between twelve and eighteen months to flatten and fade after injury but can take up to twenty four months. While there will always be some evidence of the injury some scars may cause more serious problems and it is worth seeking advice from your doctor, dermatologist, surgeon, nurse or pharmacist if the scar,

Grows bigger

Remains red/dark and raised and does not fade

Causes discomfort, itching or pain

Restricts movement of a joint

Causes you distress because of its appearance
