Rosacea is a chronic inflammatory skin disorder characterized by four different clinical phases predominantly affecting the central aspect of the face. The first evidence of rosacea is a persistent and intense vasodilation or flushing. In most people this condition advances to a vascular stage characterized by redness of the skin due to the congestion of the capillaries (erythema). Once triggered, it can continue for hours or days. Many people remain stable in this stage while some progress to an inflammatory stage defined by a symmetrical pattern of papules and pustules in addition to the lasting erythema. This stage can usually become a chronic condition. Some people, mainly males, can progress to the final stage defined by swelling or a distinctive hyperplasia (abnormal increase in the amount of cells), especially of the cells on the nose.

He noted that the body creates peptides to protect itself from viruses, fungi and bacteria. He hypothesizes that certain peptides called cathelicidins may be stimulated too quickly in rosacea patients. The level of the protective cathelicidins was 200% to 300% higher in rosacea patients. While several kinds of bacteria may trigger the synthesis of cathelicidins, he stated that they seem to be specially sensitive to the bacterium Helicobacter pylori, which has been associated with rosacea in some researches.

The researchers have also realized that while the peptide is inactive when it initially appears on the skin’s surface, enzymes in sweat cause it to become active. They concluded that cathelicidins may trigger the production of bumps and pimples in rosacea. This may possibly be in association with H. pylori and other bacterial stimulators.

According to Dr. Thiboutot, vascular endothelial growth factor (VEGF), a controller of blood vessel proliferation that may be associated to solar exposure, is highly present in the sebaceous glands of people with rosacea (5º Annual Rosacea Research Workshop, 2004). She noticed that previous investigation had found that VEGF may be implicated in the formation of visible blood vessels in rosacea, and that rosacea patients may therefore be particularly delicate to sunlight exposure.

In a further study, researchers found that neuropeptides -- substances produced by nerves or other cells in the skin -- and ultraviolet B (UVB) radiation (found in sunlight) seem to stimulate the release of VEGF. Dr. Richard Granstein, director of dermatology at Cornell University, reported that small amounts of UVB, a type of ray that causes sunburn, seems to increase the release of VEGF in endothelial cells -- that is, cells that line blood vessels.

In addition, UVB caused an increase in interleukin 8, which plays a role in inflammation. The peptide somatostatin, however, had an impeding effect on interleukin and on VEGF production in certain skin cells called keratinocytes. The relation to rosacea is still being studied.

A study done in 2003 demonstrated that in the culture of human vascular endothelial cells, snail GAGs at 10 and 100 micro g ml(-1) impeded VEGF-induced capillary tube generation.
